BSA REQUIRED MEDICAL FORMS

BSA Medical Forms are required of all Scouts in the Troop. These forms are needed for the health and safety of every Scout during Scouting events, campouts, and summer camp. As recharter is approaching, all Scouts must complete their BSA Medical Forms to re-register with the Troop.

Please provide:

• two copies of the signed BSA Medical Form (one for Camp, and one for the Troop) as well as
• two copies of the FRONT & BACK of the insurance card.

• Select the download from “Are You Going to Camp?“ so that you get Parts A, B1, B2, and C. Part C must be signed by a physician
